I still think that Dean's hallucination of Evil Sam wasn't what he thinks will happen, but the illness distorting and amplifying what he most doesn't want to happen, taking his underlying fears of where Sam's powers come from and what the YED intended them for and tangling them up with his abandonment complex and concerns over Sam's deception until you end up with this terrifying mess.
